The Medicare Coordinated Care Demonstration, authorized by the Balanced Budget Act of 1997, targets beneficiaries with chronic conditions that represent high costs to the Medicare program, such as asthma, chronic lung disease, diabetes, heart failure and related cardiac conditions such as hypertension, hyperlipidemia, coronary artery disease as well as other chronic health conditions.

The demonstration was implemented in April 2002 and was scheduled to run for 4 years. Recently CMS granted a two year extension to 11 of the 15 program sites, including Health Quality Partners, which will extend the study until March 2008. HQP has enrolled 1,732 beneficiaries in eastern Pennsylvania from 8,373 referrals. Presently HQP is collaborating with 53 physician practices that include approximately 125 primary care physicians who are caring for patients participating in the study. The HQP Coordinated Care program is a comprehensive model of care that utilizes a sophisticated risk stratification process and incorporates best practices in both geriatric care and disease management programs. The strength of the HQP model allows for both the effective and efficient delivery of a wide range of services to a large and diverse medically at-risk Medicare population.
